Dear Southern District Health Board,

I am looking for the following information omitted by your recent report on adverse events at the SDHB:

1. In 2013/14, 2014/15, and 2015/16, how many people were admitted or treated by medical staff for a suicide attempt or severe self harm event at Dunedin Hospital who had been in contact with Emergency Psychiatric Services within one month prior?

2. In 2013/14, 2014/15, and 2015/16, how many people were admitted or treated by medical staff for a suicide attempt or severe self harm event at Dunedin Hospital who had been in contact with Emergency Psychiatric Services within one week prior?

3. In 2013/14, 2014/15, and 2015/16, how many people were admitted or treated by medical staff for a suicide attempt or severe self harm event at Dunedin Hospital who had been in contact with community mental health teams within one month prior?

4. In 2013/14, 2014/15, and 2015/16, how many people were admitted or treated by medical staff for a suicide attempt or severe self harm event at Dunedin Hospital who had been in contact with community mental health teams within one week prior?

Please present this information in a table or spreadsheet for simpler interpretation by the public. Failure to respond within the required time period under the Official Information Act will result in me contacting the ombudsman.
