Clarification of patient satisfaction survey for Compass meals

Dear Ministry of Health,
Commissioner Richard Thompson stated that 'a survey of SDHB patients showed satisfaction with meals in July was over 90%.' (ODT 3 Sep 2016).
I would like to request to see the results of that survey and a copy of the questionnaire to determine:
What questions were asked?
Who asked the questions?
How many patients were asked for their feedback?
