Dear Ministry of Health,
As Guardian of the subject person, I am obliged to seek any and all avenues to determine the facts surrounding the vaccine known universally as SV40, the administration of and resultant effects on recipients. Also I require the name of the ChCh law firm that advertised in the New Zealand Herald, for those who suffered as a result of receiving the faulty Salk Polio vaccine to apply for compensation, approx 10 years back.
The vaccine was administered in 1956, and I have documented evidence to support, but require authentication of requested details by yourselves to authenticate and support my pending claim. The claim is sitting in my lawyer's office awaiting your support to formalize.

Your request has been received and logged.  You can expect a reply no
later than 8 September 2016 as required under the Official Information Act
1982.

Please note that a working day is defined in the Act as ‘any day of the
week other than a Saturday, Sunday, Good Friday, Easter Monday, Anzac Day,
Labour Day, the Sovereigns birthday, Waitangi Day and a day in the period
commencing 25 December in any year and ending 15 January in the following
year.

If a large amount of information has been requested or if the Ministry
needs to consult in order to make a decision, we may need to extend this
date (this is provided for in section 15A of the Act).  We will advise you
if such an extension is necessary.

Dear Martyn Goddard,

In relation to your request (H201603249) we are not able to answer your
query regarding the name of the Christchurch law firm that advertised in
the New Zealand Herald about ten years ago as the Ministry does not hold
this type of information. In relation to the query about the Salk Polio
vaccine (or SV40)  -  we have consulted teams in the Ministry and as the
Medicines Act only came into effect in 1981 we do not hold information
that goes back to the 1950s. I recommend seeking this information from
Archives New Zealand.
