On 18 December 2015 Lee M. made an OIA request through the FYI site for which the thread can be found at the following web address;

I have been following Lee M's request for this information and was disappointed to not be able to have access to your response Jane Foster from the FYI website.

It would be greatly welcomed if you could please post me a full copy on FYI, or alternatively email me a PDF copy to my email address at [email address]

I apologise for the problem with the PDF on the request referenced above. The problem is entirely on our end, not the Privacy Commissioner.

Our software currently has an issue handling some PDFs which contain both scanned documents (images) and OCR (scan-to-text). The document is still readable with the "View as HTML" link.
