Can you please provide all of the Medsafe / Pharmac approved medicines in New Zealand that have been responsible for one or more death in the last 15 years, and can you please provide if they are still in use. Please also provide the reporting that meant the medicine was retained, or removed from use. Please provide in the following format.

Year - Medicine - Total Deaths - Still in use - Yes or No

The information you have requested is not held by the Ministry. This
information is only held when a report is made to Medsafe, the Ministry or
the Centre for Adverse Reactions Monitoring (CARM) at the University of
Otago. It is not always known if the death was in any way related to an
approved medicine, a combination of a side effect and poor treatment, or
for example, an interaction with a food.  

 

A summary of reports of suspected adverse reactions to medicines can be

of reports stating the person had died is stated in the output. All
