Recordkeeping problems

Different records are being given to patients and clinicians in their care. Staff have told patients this happens. Professional conduct sees community GPs working with their hospital colleagues in good faith, and by that standard they expect paperwork and patients aren't screwed with. In addition, there's been some cases where falsified records are entered. Good medical practice mitigates agency conflict and maximises patient welfare, and in the public system it's supposed to be at the best price for the most people. Worse still, there's evidence staff are manipulating each other through some patients records and this isn't in their best interest.
What's going on and what's it costing? Please provide related documentation and figures.
