Complaints against Dr Alan Farnell

Dear Health and Disability Commissioner,
I am seeking information in regards to;
1. the number of complaints that have been lodged against Dr Alan Farnell in the last 10 years (since 2011), and;

2. if he was censured or had any restrictions imposed on his license at any time in the last 15 years (2006) the documents showing the outcomes with private information about the complainant redacted. I'm just looking for the outcome on Dr Farnell.
