Dear Health and Disability Commissioner,
In reference to the story of a woman commiting suicide while waiting for an urgent assessment, posted on October 19th, by NZHerald (https://www.nzherald.co.nz/nz/suicide-fa...)
Please provide any documentation, correspondance, notes, meeting minutes, etc regarding;
* Investigations as to whether other DHBs had sufficient standards, protocols, etc that would avoid this happening in their DHB
* Whether any corrective actions were made at the other 19 DHBs as a direct result of this case.
