Dear Ministry of Social Development,
Under section 12 of the Official Information Act, I hereby respectfully request any and all information pertaining to the number of persons formerly on the sickness benefit who have since died during the transition period to the new jobseeker support allowance.
I also request any information the ministry may have about the impacts of this transition from sickness benefit to jobseeker allowance on the mental and physical health of the beneficiaries in question.
