Breaches of IDI policy

Dear Statistics New Zealand,

Under the Official Information Act, we request the following information since the establishment of the Integrated Data Infrastructure (IDI) to the date of this request:

1. Each instance where an individual has breached a policy on access to the IDI. In each case, please identify:
(a) The date or date range that the breach occurred;
(b) The policy which was breached;
(c) Whether the individual was an employee of Statistics New Zealand, a contractor to Statistics New Zealand, or a representative of a third party organisation;
(d) If the individual was a contractor or third party, the name of the contractor or third party;
(e) If the individual was an employee of Statistics New Zealand, whether the individual is still employed by Statistics New Zealand;
(f) The time interval from the start of the policy breach to its discovery by Statistics New Zealand;
(g) The action taken by Statistics New Zealand against the individual and/or third party as a result (such as revocation of access, discliplinary action, or prosecution, or any other action taken).
(h) If known, the purpose of the breach.

2. Any reports, memoranda, or aides memoire Statistics New Zealand has provided to a Minister of Statistics (past or present) regarding the breaches above, including the date it was provided;

3. Any other internal reports or memoranda prepared by Statistics New Zealand as a result of any of any IDI policy breach.
