Request for a fll copy of ACC's Reasons to revoke cover policy document

Dear Accident Compensation Corporation,

I refer to your recent FYI response (https://fyi.org.nz/request/5859/response...) to Mr A Jordan on 9 June 2017 replying to his OIA requests dated 11 May 2017.

Request 1:

Within the ACC policy document titled "Timeframes to determine cover" supplied by ACC through FYI to Mr Jordan, under the Introduction, on the first page of this document, highlighted in blue are the words "deemed in their favour".

It appears that the highlighted words in the policy document allow an ACC employee to link off this issue in blue to establish either the definition of what is highlighted in blue, ACC statutory wordings and or further ACC policy documentation.

It would be appreciated if you could supply me with the official information that is linked to and can be accessed by an ACC employee when they click on the wording "deemed in their favour" as noted above.

Request 2:

Within the ACC policy document titled "Deemed cover decisions when timeframes not met" supplied by ACC through FYI to Mr Jordan, under the Introduction, on the first page of this document, highlighted in blue is the word "timeframes".

Request 3:

Within the ACC policy document titled "Deemed cover decisions when timeframes not met" supplied by ACC through FYI to Mr Jordan, under the second bullet point in section "Rules", on the first page of this document, it is written "See Reasons to revoke cover" in which "Reasons to revoke cover" is highlighted in blue, denoting further ACC policy documentation"

It would be appreciated if you could supply me with a full copy of the official policy documentation and or information that is titled "Reasons to revoke cover" as noted above.

Request 4:

Please provide the official information on what ACC define the following terms highlighted in blue in the policy document titled "Deemed cover decisions when timeframes not met"

(a) "converted weekly compensation", and

(b) "revoke",

and provide a copy of the full information an employee of ACC would find if they linked of either of these highlighted tabs.

I respectfully request pursuant to section 12 subsection (c) of the Official Information Act that this information is provided to me under urgency within 5 working days, being by the end of business close on 4 July 2017 for the following reasons;

1. The information is readily held by ACC, and therefore readily accessible for supply.

2. This information is required so that I can formalise my request for assistance from the Office of the Ombudsman for ACC having not complied with their statutory obligations to have provided me with decisions on cover, nor having had provided to me as statutorily required to, written confirmation of deemed covers for classes of injury covers that I have formally lodged claims for under sections 48 (a), (b) and (c) of the ACC Act under several claim numbers during 2015, 2016 and 2017.

Under s16(2) of the Official Information Act, my preferred way of receiving a response is by email to the address from which ACC received the original request, not by post"
