Under the Official Information Act 1982, I request:
The consent form or process used to obtain client consent for sharing sensitive health information with MSD Case Managers, any guidance provided to clients about what health information will be shared and with whom, any assessment of whether MSD's health information collection practices comply with the Health Information Privacy Code 2020, and any complaints received about health information privacy in the DA process (number per year, 2020-2025).
All operational guidelines and staff instructions for processing Disability Allowance claims for non-Pharmac-funded medications, including: what evidence is required from the treating practitioner, what additional evidence (beyond the treating practitioner's certification) can be requested, who determines whether the additional evidence requirement is satisfied, and the clinical qualifications (if any) required to make this determination.
Any policy requiring PHA review of decisions to decline applications certified by registered medical practitioners.
