Dear Sir
I am writing to make an official information request under the Official Information Act 1982.
My request is for information about the service delivery contracts the Ministry of Social Development holds, and has held with the Wairarapa branch of the Open Home Foundation.

I am requesting this information because I have been advised that the Wairarapa branch of the Open Home Foundation very recently lost its CYF contract for service delivery from the Ministry. I wish to establish the reason/s this happened to determine if this is in any way related to my concerns about the professional practice of staff employed by the Open Home Foundation to work with children in the custody of the Ministry.

The information I am requesting is as follows:
1. What service delivery contract/s do/did the Ministry hold with the Wairarapa branch of the Open Home Foundation in relation to Child Youth and Family and Community Services?

2. What service delivery contract/s are either not being renewed, and/or or have been terminated within the past 12 months?

3. A copy of the service delivery contracts that are either not being renewed, and/or have been terminated within the past 12 months?

4. What are the quality performance measures for the service delivery contracts that are either not being renewed, and or have been cancelled within the past 12 months?

5. What is/has been the performance against the service delivery requirements of the contract/s the Ministry holds/has held with Wairarapa branch of the Open Home Foundation over the past 12 months?

6. What are/were the reasons the Ministry did not either renew or have decided to terminate these contracts?

Given I am requesting this information because I remain concerned about a child’s safety, could you please treat my request for this information as urgent, and expedite your response.
