Dear Waikato District Health Board,
Please can you provide information to the amount of instances that WDHB is aware of where staff have accessed patient records without any obvious cause.
Please include whether any disciplinary action was taken and if relevant how many staff were given formal written warnings and how many staff had their employment terminated
