The purpose of this document is to create consistency in the processing of Standard Terms section 241 
applications, showing NZ Transport Agency Waka Kotahi (NZTA) what types of responses we are seeking 
from the applicant. 
1. Company and Registration Details

Company Name: Legal name, verified against the Companies Register.

Trading As Name: Other names the company is known by, for authorization clarity.

Company Number: Searched if not provided.

Motor Vehicle Trader/Financial Service Provider Number: Used to verify registration status.

NZTA Customer Number: Indicates history with NZTA

Motochek Account Number: Shows if the applicant uses Motochek, guiding next steps.

Nature of Business: Confirms business type.

Business Locations: Addresses, verified for official records.

Business Operating Duration: May prompt further questions if the business is established but
new to authorization.

Company Contact Details: Preferably both a specific person and a shared inbox.

Staff/Agents with Register Access: Checks for anomalies in staff numbers.

Roles of Those with Access: Common-sense review of appropriate roles.
2. Training and Handling of Information

Training Provided: Details, consistency, materials, record-keeping, staff sign-off, use of Office of
the Privacy Commissioner (OPC) modules, documentation, real-world scenarios, frequency.

Controls for Proper Handling: Confidentiality measures, policy references, training schedules,
onboarding/offboarding, login management, retention/deletion policies, record-keeping evidence.

Disciplinary Measures: Types of warnings, possible termination, retraining requirements.

Current Register Use: Whether and how the register is used.

Intended Use of Register: Specific purposes for accessing information.

Reasons for Not Col ecting Information Directly: Why direct collection isn’t possible.

•  Reasons for Not Using Free Online Service: Suitability of NZTA’s free service. 
•  Reasons for Not Using OIA Process: Timeliness concerns with Official Information Act requests.  
 
4. Notification to Individuals 
•  How Individuals Are Notified of Register Use: Must specify written notification methods. 
•  How Individuals Are Notified of Information Use: Details required. 
•  How Individuals Are Notified of Opt-Out Option: Must explain how people can opt out of having
