i) ordered the product
ii) imported the product; and
iii) the product was charged to
From March 2020 until July 2023 (inclusive) please provide for all imports
Copies of the
-purchase order form
-shipping form and/or waybill shipping
-Packing slip for product on arrival
MBIE understands that you have made the same request to the Ministry of Health and Pharmac. MBIE 
does not hold any information on this matter beyond what is likely to be held by those agencies.
