Thank you for your email on 30 April 2025, asking Health New Zealand | Te Whatu Ora for the
following under the Of icial Information Act 1982 (the OIA):
Total number of inpatients under the respiratory team (General Medicine) at Auckland City
Hospital on 1 May 2025.
Number of those patients reviewed by an SMO by 5pm.
For patients not reviewed by an SMO, the level of doctor who reviewed them.
Number of MET calls or ICU transfers involving respiratory patients on that day.
Copies of any incident reports filed involving respiratory patients between 30 April and 2
May.
Any records (emails, rosters, meeting notes) indicating staff were pressured or directed to
work on 1 May despite ASMS strike action.
Response
1. “Total number of inpatients under the respiratory team (General Medicine) at Auckland City
Hospital on 1 May 2025.”
Please note that the Respiratory Service does not sit within the General Medicine Service
There were 13 inpatients admit ed under the Te Toka Tumai Respiratory Service at 00:00 on 1
May 2025.
2. “Number of those patients reviewed by an SMO by 5pm. For patients not reviewed by an
SMO, the level of doctor who reviewed them.”
It is usual practice for all respiratory transplant inpatients to be reviewed by an Senior Medical
Officer (SMO) daily. On 1 May 2025 all respiratory transplant patients were reviewed by the SMO
covering Life Preserving Services (LPS).
It is usual practice that general respiratory patients are reviewed by an SMO within 24 hours of
admission and then every two - three days unless there are concerns. This was the case on
1 May 2025, with all patients not reviewed by the LSP SMO being reviewed by the Resident
Medical Officer. The patients that were not reviewed by an SMO on 1 May 2025 were to be seen
by an SMO the following day.
It is not possible to provide the exact numbers of those patients reviewed by an SMO by 5pm on
1 May 2025 without conducting a manual audit of individual patients’ notes. To do this
Health NZ would need to divert clinicians from their core duties of caring for patients to undertake
an audit. As such, and given the priority of clinicians’ work, we are refusing your request under
substantial collation and research. The diversion of these resources would impair Health NZ ability
to carry out its core functions. Your request is therefore refused under section 18(f) of the Act.
I have considered whether fixing a charge for the supply of the information or extending the
timeframe for response would enable Health NZ to respond. I do not consider that either option
would remove the impact that supplying the information would have on our operations.
3. “Number of MET calls or ICU transfers involving respiratory patients on that day.”
There were a total of eight ‘Patient at Risk’ encounters for the day. There are no respiratory
admissions to Critical Care.
4. “Copies of any incident reports filed involving respiratory patients between 30 April and 2
May.
I note the general expectation that medical records wil be kept confidential and that patients can
rely on Health NZ / Te Whatu Ora not to create the risk that they wil be identified. There were two
incidents reported over this timeframe. However, this information has been withheld under section
9(2)(a) to protect the privacy of natural persons.
5. “Any records (emails, rosters, meeting notes) indicating staff were pressured or directed to
work on 1 May despite ASMS strike action.”
We are not aware of any such emails, rosters, meeting notes, or communications indicating that
staff were pressured or directed to work on 1 May 2025 despite ASMS strike action.
Your request for this part of information is refused under section 18(e) of the Act as this information
does not exist or, despite reasonable ef orts to locate it, cannot be found.
