Thank you for your email on 17 December 2024, asking Health New Zealand | Te Whatu Ora for
the fol owing under the Official Information Act 1982 (the Act):
“Please provide copies of:
- The risk management policies and procedures for Te Whatu Ora.
- Copies of the most recent risk registers for each of the four regions.
- Copies of the most recent risk registers for each NZ hospital.”
On 19 December 2024, Health NZ contacted you to refine your request, otherwise it would be
refused under section 18(f) of the Act as substantial manual col ation would be required to provide
the information requested. On 20 December 2022, you confirmed that you would like to proceed
with the fol owing refinement:
“The national and four regional strategic risk registers’ as wel as an ‘outline [of] any other
registers that exist, but not provide the actual registers, at this stage.”
Response
Your request for information, even though it has been refined is very broad, and substantial manual
col ation would be required to locate and prepare al documents within its scope. As such,
providing al of the information requested would ‘impair efficient administration’ under section
16(2)(a) of the Act.
However, we have provided contextual information in response to your request, under section
16(1)(e) of the Act.
Health NZ along with other agencies, such as the Ministry of Health | Manatū Hauora,
Pharmaceutical Management Agency (Pharmac), and the Health Quality and Safety Commission |
Te Tāhū Hauora, are responsible for ensuring public funding and services protect, promote and
improve the health of al New Zealanders (section 3 of the Pae Ora (Healthy Futures) Act 2022
(Pae Ora Act)).
These agencies along with others are also responsible for considering or implementing policies
and procedures to ensure improved hauora outcomes for New Zealanders as wel as identifying
and managing risk.

and policies. The Ministry also regulate and monitor the health system.
