Thank you for your request made under the Local Government Official Information and Meetings 
Act 1987 (the Act), received on 29 May 2023. You requested the fol owing information: 
•  Is, or has, the Council conducted a review into the failings that led to the installation of 
faulty streetlights? If so, can you please provide a copy of its scope? 
•  I want to find out if the review wil  consider the process used to appoint people who 
worked on the project and whether the people working on the streetlight project were 
appropriately qualified. 
 
The Council has conducted a review into the failing streetlights.  This review has been completed 
and is publicly available.  Please see proactive release in particular items (3-6) related to testing of 
the adaptor. In late 2018 Gess Limited undertook an assessment to find cause of the fault.  Another 
review was undertaken in February 2023 by WSP to test the adaptors. 
 
The Council’s Assurance and Business Integrity team also reviewed the streetlight  failure. The 
outcome of their review is publicly available.  Recommendations from this report include: 
•  That the Council has controls for design sign-off including: 
o  guidelines on who can sign off designs and under what conditions, 
o  a record of who signed-off on a design, their qualifications, and a one-up manager 
approval. 
•  Improved health and safety management by: 
o  Ensuring all staff are appropriately trained in responsibilities for reporting near-miss 
and events for al  staff or public.
