Decision on information request  
In relation to your request dated 1 December 2022, for the fol owing information: 
1.  Provide process documents for TAIC investigations of an incident.  
TAIC Investigations progress through a series of phases per TAIC’s investigation 
methodology. Each phase has a discreet set of tasks that enables us to make evidence-
based findings that have been comprehensively reviewed and evaluated. The 
investigation process has eight phases. An overview of each phase and the tasks 
associated with them are presented in the attached document.

2.  Does TAIC assess to see if the operator suffered a sudden medical event prior to the 
incident such as cardiac, stroke, or clot?  
The Commission considers medical events as a matter of course in al  investigations. For 
example, post-Mortem results are obtained routinely when there is a fatality of a person 
involved in a safety-critical role. The Commission employs a medical consultant 
specialising in transport safety to review al  medical evidence gathered as part of the 
inquiry, the review may result in a finding or recommendation if we believe it may lead to 
improvements in future transport safety.
