Add an annotation to “Decisionmaking on passport application form”

• Thank the public authority or Phil Lyth.
• Suggest how the requester can find the rest of the information.
• Point to related information, campaigns or forums which may be useful.

Annotations will be posted publicly here, and are not sent to Department of Internal Affairs.
