Dear hnzOIA,
Mindful that there is a legislated timeframe, relying on an automatic reply will cause you to be outside of the required timeframe
Yours sincerely,

Thank you for your request for official information, received on 3
November for:
I am a New Zealand citizen and reside in New Zealand. Under the OIA, I
request a copy of the following
For each of the current localities/ex DHB regions (Health NZ only not NGO)
1) Copy of the treatment plan template (myplan/wrap/shared care
plan/treatment plan) used within each mental health service
2) Description and number of mental health and addiction lived experience
roles (CPSLE)
After some initial discussions within National Office, the information you
have requested does not appear to be held centrally.
While some of this information may be held at the local level, your
request would require a significant amount of collation across 20 Te Whatu
Ora districts and Te Whatu Ora would need to divert personnel from their
core duties. Doing so would be administratively challenging and would
require extensive consultation as regions hold data in different ways.
There are efforts to standardise data across the country, but these
efforts will take time.
In the interests of providing you with accurate information in a timely
manner, would you consider amending the scope of your request?
To assist you, we have provided examples below of information we hold that
we believe would satisfy your request:
* limit to single Te Whatu Ora local health area; and/or
* limit to information that we can provide at a national level, noted
above
We appreciate you are under no obligation to amend your request, but
requests that require substantial collation may refused under section
18(f) of the OIA. We want to work with you to avoid this and ensure we
are able to provide a response that best meets your needs.
Te Whatu Ora has already received your other OIA request, HNZ00032042, if
you just wanted to continue with this request if it was the Lakes region
you were interested in.
1) A list of Mental Health & Addictions Consumer, Peer Support and Lived
Experience Roles within Te Whatu Ora Lakes region/locality and a
description of what these roles are expected to provide.
2) A list of Mental Health & Addictions Consumer, Peer Support and Lived
Experience Roles in the lakes region that are funded by Te Whatu Ora
(within the NGO sector)
Please let us know before 17 November 2023 whether you are prepared to
amend or clarify your request and, if so, how.
Please note, under section 15 of the Official Information Act 1982, any
clarification or amendments made to a request within seven days after the
date it is received, that request may be treated as a new request and the
time limit for the response restarts.
We look forward to receiving your response.
Ngâ mihi
Vicky

Dear hnzOIA,
happy to amend it to just
1) Copy of the treatment plan template (myplan/wrap/shared care plan/treatment plan) used within each mental health service (secondary service - cmh ect)
It should not be too difficult, every specialist service should be very familiar with their own treatment plan
Yours sincerely,
Anatoly

Thank you for refining your request, sorry for our delay in confirming
that we are working on this request.
Because this request was refined the date due out back to you changed to
be the 13 of December, resetting the clock from the date you replied to
our email, we are currently still working on providing you a response for
this. For this request we need to consult with each of the regional
departments throughout New Zealand and this can take some time.
thanks, Vicky

Tēnā koe Anatoly,
Thank you for your email of 3 November, asking for the following
information under the Official Information Act 1982 (the Act):
I am a New Zealand citizen and reside in New Zealand. Under the OIA, I
request a copy of the following
For each of the current localities/ex DHB regions (Health NZ only not NGO)
1) Copy of the treatment plan template (myplan/wrap/shared care
plan/treatment plan) used within each mental health service
2) Description and number of mental health and addiction lived experience
roles (CPSLE)
Which was later refined to
1) Copy of the treatment plan template (myplan/wrap/shared care
plan/treatment plan) used within each mental health service
This email is to let you know that Te Whatu Ora needs more time to make a
decision on your request.
The Act requires that we advise you of our decision on your request no
later than 20 working days after the day we received your request.
Unfortunately, it will not be possible to meet that time limit and we are
therefore writing to notify you of an extension of the time to make our
decision, to 31 January 2024
This extension is required because the consultations that are necessary to
make a decision on your request are such that a proper response cannot
reasonably be made within the original time limit. For this request we
need to consult with a significant number of local areas throughout New
Zealand. This takes time but we are actively working on it and will
provide the response as soon as we can.
If you have any questions, please contact us at [1][Health New Zealand request email]
If you are not happy with this extension, you have the right to make a
complaint to the Ombudsman. Information about how to do this is available
at www.ombudsman.parliament.nz or by phoning 0800 802 602.
