Dear Ministry of Social Development,

Under the Official Information Act I wish to know the following;

1.What is the total number of suicides of parents,children and caregivers involved with CYF?
2.What is the total number of children that have died or have been seriously injured(accident or otherwise)under CYF care since records began?
3.What is the procedure for CYF when informing family of a death/serious injury? What is the procedure in regards to caregivers (funeral,hospital contact etc)?
