In regards to part two of your request for the names (and chairs) of any Royal Commissions 
and  Commissions  of  Inquiry  held  during  the  1990s,  DPMC  will  provide  a  further  letter 
responding to this part of your request by 5 June 2019. 
In regards to part three of your request,  DPMC does not hold information relating to inquires 
chaired by Helen Cull QC however, I believe the Ministry of Health and the Fire and Emergency 
Service may  hold this information.  Accordingly,  I have decided  under section 14(b)(i) of the 
Act  to  transfer  this  part  of  your  request  to  the  Ministry  of  Health  and  Fire  and  Emergency 
Service. 
You have the right to ask the Ombudsman to investigate and revi13w my decision under section 
28(3) of the Act.
