I  am  writing  to  acknowledge  receipt  of  your  email  dated  24  April  2018  requesting  the  following  information 
under the Official Information Act from Canterbury DHB.  
 
Many of our people in the community undergo treatment for gallstones and cholecystitis 
 
Treatment options includes elective gallbladder surgery commonality known as cholecystectomy 
(laparoscopic cholecystectomy) 
 
Please provide a copy of ALL the information you provide to clients (public heath consumers) during the 
clinical assessment, cholecystectomy (laparoscopic) preoperative informed consent and surgery admission 
process. 
 
Information to include but not exclusive to: 
 
• 
Patient treatment information handout / fact sheet brochures  
• 
Patient informed consent form or forms  
• 
Elective surgery consent form or forms  
• 
Post cholecystectomy wound care, recovery and diet information  
• 
And any other handout information provided to clients 
 
Include also if possible published and revision dates 
 
We will endeavour to respond to your request as soon as possible and in any event no later than 24 May 2018, 
being 20 working days after the day your request was received, and taking into account the public holiday for 
ANZAC Day. If  we are  unable to respond to your request  by then, we will notify  you of an  extension of that
