I attended the incident at the request of the incident controller, in accordance with 
Operational Instruction P3.

A short-duration, apparently high-intensity fire had occurred in a structure used for the 
dissolution and disposal of solid chemicals with water.  The open end of the structure, 
measuring approximately 7.5 metres by 15 metres included two similarly sized bunded 
³pits´made of concrete block and concrete render.  
The pit involved in fire in this incident was lined with PVC material.  Fire had consumed 
much of this material in the rear half of the pit.  There were signs of heat damage to 
roofing iron above the rear half of the pit, and to metal ventilation ducting running above 
the pit and along the adjacent wall of the structure.
Released 
Operators of the site advised that the process being undertaken at the time of the fire 
included the distribution of powdered SODIUM CHLORITE (chem NaClO2) compound 
into the pit in preparation for dissolution.  The material had been stored in 50kg nett 
metal drums, which had been in turn lined with plastic, in accordance with instructions 
contained in relevant MSDS documentation (copies downloaded).
The process involved one worker bringing individual drums to the side of the pit from the 
pallet upon which about 25 drums were stacked.  It was noticed that these drums all 
displayed significant extent and depth of oxidation (rust) on their outer surfaces.
The second worker was located in the pit (measuring approximately 1100mm deep), 
whose function was to open the containers and empty the contents.

It was reported that in the process of emptying the first of the drums, the worker noted 
that a quantity of the material had compacted at the base of the drum, and attempted to 
loosen this material with a wooden ³broom handle´It was during or shortly after this 
action that the worker noticed ³sparking´among the material either in the drum or on the 
floor of the pit or both, and decided to evacuate the pit.  An explosion and fire occurred 
shortly after that.
The nature of Sodium Chlorite is that of an active oxidiser, which will react actively in the 
presence of organic compounds and other oxidisers (according to MSDS).  The reaction 
of this material is exothermic in nature, and will generate sufficient heat to reach auto-
ignition temperature of the material in a short period of time.  The burning of the material 
is accelerated by the release of oxygen as part of the reaction process.
It is stated that the material is not reactive to percussion or shock, and it is unlikely the

action of stirring and loosening the material with a wooden broom handle would have 
generated sufficient heat or impact to result in the commencement of a chemical 
reaction.
The indicated cause of the ignition of the material is as a result of the contamination of 
the material during the decanting process, with an organic compound or other oxidising 
agent.  It was noted that there were a number of containers stacked at the edge of the 
pit, including one marked ³Red Nitric Acid´which is a volatile oxidiser.  
The precise nature and method of introduction of the contaminant material is unknown.  
The statement from the operator of the plant indicate a cleaning routine using water was 
applied to the pit prior to the incident, this tends to be contra-indicated, with the 
contamination of the material occurring shortly after the first load was introduced to the

pit.  The reaction of the wooden broom handle with the oxidiser was considered, 
however this could not be conclusively confirmed as being the source of contamination.
The indicated category of cause of fire is Accidental.  Fire has originated in the base of 
the pit being used for dissolution, as the result of an introduced contaminant reacting 
with the oxidising material.  The compacted material in the base of the drum has ignited 
and burned rapidly and explosively.  A runaway exothermic reaction ignited the PVC 
lining of the pit and the Sodium Chlorite.  Fire has developed rapidly, with the assistance

of oxygen released as part of the chemical reaction.  The consumption of available fuel 
(PVC) was fairly rapidly affected.  The absence of further nearby fuel loads was a factor 
in the rapid containment of the fire.

Action was taken and a full report was not created because: Waste Management also 
conducting their investigation.  No details of their findings have been forthcoming as yet.
