Official Information Act Request 
Thank  you  for  your  request  of  25  May  2017,  asking  for  the  following  information 
under the Official Information Act 1982 (the Act):  
 
I  respectfully  request  that  ACC  Government  Services  provide  to  me  in 
keeping  with the  Official  Information  Act  1982  PDF  copies  of  the  Treatment 
Provider  Handbook  that  was  applicable  as  at  1  January  2012,  and  PDF 
copies  of  all  subsequent  versions  through  until  the  2015  version  which  was 
applicable through to 2016 which ACC Government Services provided to me 
on 24 May 2017.   
 
Our response 
Please  find  enclosed  four  Treatment  Provider  Handbooks  (TPH)  which  covers  the 
entire duration specified in your request. These are labelled according to when they 
were first published, as follows: TPH 2011; TPH Oct 2014; TPH Dec 2014; and, TPH 
June 2015.  
 
Please note that the TPH provided to you on 24 May 2017, titled “TPH 2015”, was 
first published in August 2015, after the 2015 handbook supplied with this request.
