Reporter Comments
A passing Police car discovered the fire on the top floor of the car dismantling machine, 
building. The room on the second floor contained the control room for a machine that 
shreds whole cars. This room was full of electrical control equipment and included the 
operators chair and controls. A hazard, high voltage power alert was passed to 311 prior 
to arrival and on arrival it was clear that electrical wiring and associated equipment was 
involved by the copper colour in the flames and also the sparking and explosions. The 
control room and power sub and motor rooms were part of a larger building measuring 
10mx10m which included the car shredder machine. The building was secure when the 
Brigade arrived. A defensive exterior attack using 1x aerial and 1x ground monitor was 
commenced whilst the high voltage power was isolated. This was increased to two aerial 
monitors. After power was isolated a LPD supported by 1x HPD was taken up to the 
second floor to extinguish the remaining fire. Due to the complete destruction of the 
control room and removal of other possible causes an electrical control or part failure of 
undeterminable nature is the best choice for the origin of cause for this fire.

As described by SO Beech fire involved the top level of the control building of a ³car 
shredder´Only the control room of the building was involved in fire.  The control room 
was approximately 4x4 metres.  The west and south (shredder side) walls of the room 
were glass.  The room was used by the shredder operator to control the machine and 
was equipped with a number of computers on a desk against the east wall television 
monitors and a ³control station´in the south west corner of the room.  The ³control 
station´was mounted on a steel platform.  The building is situated back from the road 
and almost obscured from view from the road by elevation, other buildings and trees.  
The fire was reported by a passing Police vehicle.  At that time the room was well 
involved.  All combustible contents of the room were completely destroyed by fire.  
Initial observation of the scene during daylight the day of the fire (21/5/17)  identified that 
the point of origin was most likely in the area of the control station.  Due to the nature of 
the site including the security it was determined that investigation would be continued 
when site owners and staff were present.
At a meeting of the site principles (22/5/17) it was determined and agreed that further 
investigation would be undertaken in concert with the insurance company investigator 
who was to provide an electrical engineer to assist.
Answers to general questions regarding the usual activities around operating the 
machinery and the activities that had occurred during the day preceding the fire 
confirmed that the machinery was operating normally with no faults noticed.  The

operator had commenced operation at approximately 7:00am and worked through until 
3:00pm.  It was confirmed that the operator had followed the specific shutdown 
procedure which includes isolating power to the system.  The computer systems were 
set to a 30 minute time out.
There were signs that rat/vermin activity was present on the site in the form of bait 
stations in the building and confirmed that there was a known issue with rat/vermin 
infestation
Asbestos was identified as a hazard on the scene which resulted in the need of the 
owners/operators of the site to manage the health risk.  At the time of meeting the 
appointed insurance investigator on site (25/5/17) the control room had been stripped of 
contents and only the concrete, steel and some of the timber framing remained.  The

electrical equipment was relocated outside the building however the relationship of the 
items could not be identified.
Observation of the timber framing around the edges of the floor and below the floor 
showed evidence of burning at a very low level with the worst area being the southwest 
corner below floor level in the vicinity of the control station.
Based on the information obtained by observation and discussions with all parties the 
most likely cause of the fire has been determined to be an unidentified electrical fault
