I refer to your Official Information Act request received by email on 8 September 2016 in 
which you specifically requested the following: 
 
Seeking information regarding numbers of children all under the age of 16, whom 
have died in the opinion of the coroner as a result of inadequate treatment and/or 
incompetent response from sdhb in the last 10 years. *I am seeking raw numbers only, 
no personal identifying information. 
 
Section 57 of the Coroners Act 2006 requires a Coroner to conduct and inquiry, not to 
determine civil, criminal or disciplinary liability but to establish , so far as is possible, that a 
person has died, the person’s identity, when and where the person died, the causes of death 
and the circumstances of the death.  Coronial findings do not express the determination of 
“inadequate treatment” or “incompetent response” on the part of a District Health Board.   
 
We are therefore not able to provide the information you requested as this is not collected as 
set out above.
