Thank you for your request for information, transferred on 2 February 2026, from the Office of Hon 
Simeon Brown to Health New Zealand | Te Whatu Ora, for the following under the Official 
Information Act 1982 (the OIA):  
Internal reporting on ED deaths, near-misses, or sentinel events linked to resource 
constraints since January 2024. 
Response 
The requested information does not exist at a national level and collating it from each district would 
require Health NZ to undertake a project of work to provide it. The OIA does not require an agency 
to form an opinion or create new information to answer a request. Your request is therefore 
refused under 18(g)(i) as the information requested is not held by Health NZ and the person 
dealing with the request has no grounds for believing it is held by another agency subject to 
the OIA.  
We have considered whether fixing a charge for the supply of the information or extending the 
timeframe for response would enable Health NZ to respond; however, we do not consider that 
either option would remove the impact that supplying the information would have on our other 
operations.
