Thank you for your email on 17 February 2026, asking Health New Zealand | Te Whatu Ora for the
following under the Official Information Act 1982 (the OIA):
I have received some meeting minutes but not all. This request for information is for several
meeting minutes, not the content of a register which was supplied when not requested.
Your response says " 43 Panel meetings were held".
When you say "Please note that no meetings were held on 16 November 2021 or 6
December 2021" , these dates are in actual fact, in the content of register in the column
"Panel assessment date" .
The dates in that register for panel assessment dates for which minutes have not been
shared are 1. November 2021 on 16,17,18,22, 24 and 29th.
2. December 2021 on 1,3,6,10,14 and 20th 3. 3. January 2022 on 11 and 18th.
4. February 2022 on 15 22 and 23th
5. March 2022 on 1st.
One would assume that each panel assessment would have meeting minutes of that panel.
The dates in my original request for information are based on the dates that are in that
column.
Under the Act, I request the other meeting minutes for dates in my previous update. If the
minutes do not exist, kindly say so. You said 'The non-confidential content from this register
was released to you as Appendix One with HNZ00035461".
If the minutes are confidential content, kindly respond to this request for information under
the act.
Response
The only formal documented minutes available from the meetings of the Significant Service
Disruption (SSD) Panel (the Panel) are from meetings on 13 November 2021, 14 November 2021,
and 15 November 2021. These were provided to you in your previous OIA request on 13 May 2024
(ref. HNZ00035461).
For all meetings after 15 November 2021, the Panel reviewed all applications and feedback was
recorded on a register, rather than in formal meeting minutes. The non-personal information from
that register was also provided to you in that same response (ref. HNZ00035461). Since there are
no formal meeting minutes available for meetings between 17 November 2021 and 23 August
2022, as you requested, the register was provided to you.
Please note that there is no record of the Panel meeting on 16 November 202 or 6 December
2021, while there are applications listed as assessed on those dates. It is likely that those
applications were initially scheduled for assessment at that date but were then reviewed at later
meetings after more applications were received.
