Response to your request for official information
Thank you for your request under the Official Information Act 1982 (the Act) to the Ministry of
Health – Manatū Hauora (the Ministry) on 2 December 2025 for:
1. Would you please share how many children in NZ have died after and because of
receiving COVID-19 vaccination from the adverse event reporting system and from the NZ
Immunization database?
We interpret your request to mean a death that was reported to the Centre for Adverse Reactions
monitoring (CARM) that was considered to have been caused by a COVID-19 vaccination. As
you are aware, anyone can report adverse reactions to CARM. However, a cause of death of this
nature must be determined by the coroner One death of an individual aged under 18 years was
reported to CARM, where the coroner found multiple possibilities for the cause of death, of which
one was COVID-19 vaccination. This case was made public by the coroner. To respond to this
question, the Ministry also contacted Health New Zealand to see it it held information in the
immunisation database, however, they advised that they do not have data linking deaths to
vaccination records.
2. Would you please share your correspondence, memos, minutes or any other discussions
of this particular CDC report by Vinay Prasad MD MPH, CBER Director, CMSO US FDA or
other briefs on this report?
No such information exists. As such, this part of your request is refused under section 18(g)(i) as
the information requested is not held by the Ministry and there are no grounds for believing it is
held by another agency subject to the Act.
If you wish to discuss any aspect of your request with us, including this decision, please feel free
to contact the OIA Services Team on:
[email address].

Under section 28(3) of the Act, you have the right to ask the Ombudsman to review any decisions
made under this request. The Ombudsman may be contacted by email at:
[email address] or by cal ing 0800 802 602.
Please note that this response, with your personal details removed, may be published on the
