Thank you for your email of 6 November 2025, asking for the following information under the Official
Information Act 1982 (the Act):
The information I request relates to when ACC make an electronic request for clinical records to
a claimant’s GP or treatment provider and the provider provides the information electronical y.
- where are these clinical records stored?
- are they given a file reference?
- when requesting a file copy (claim file) and party file are ACC required to release the clinical
records as wel ?
Our response Clinical records are uploaded and stored in EOS, which is ACC’s main claim management system that
holds client and claim information. It also holds client payment and treatment information. Records
are stored on the claim file that the requested information relates to.
A copy file is a copy of everything we have on file for the claim(s) requested. If there are clinical notes
held on the requested claim(s), these would be included as part of the copy file.
As this information may be of interest to other members of the public
ACC may publish a copy of this response on ACC’s website. All requester data, including your name
and contact details, will be removed prior to release. The released response will be made available
