The Committee questioned the implications of pausing the incident investigation process and sought clarification
that incident investigations are still occurring. Leads clarified that the pause in the incident investigation process
refers only to the technical system solution, not to the investigation of individual incidents, which continue as
required.

Leads noted that the Health, Safety, and Wellbeing Roadmap is being reviewed, with a focus on regulatory
requirements and de-escalation training for staff in higher-risk roles. The committee questioned the adequacy of
de-escalation and safety training for staff working alone or in higher-risk environments.
