Thank you for your email received by Health New Zealand | Te Whatu Ora on 18 August 2025,
asking for the following under the Of icial Information Act 1982 (the OIA):
Please provide copies of all warnings, advisories, and information given to individuals
prior to receiving any Covid-19 vaccination in New Zealand. This includes but is not
limited to written materials, consent forms, verbal scripts, and any other documentation
outlining risks or precautions related to the vaccine.
Reported Side Effects to CARM: Please provide a comprehensive list of all side effects
and adverse events reported to the Centre for Adverse Reactions Monitoring (CARM) in
relation to Covid-19 vaccines administered in New Zealand. For each side effect, please
include:
The name or description of the side effect/adverse event
The number of reports received for each effect
The time period covered by the data

For the sake of clarity, I wil address each question in turn.
Please provide copies of all warnings, advisories, and information given to individuals
prior to receiving any Covid-19 vaccination in New Zealand. This includes but is not
limited to written materials, consent forms, verbal scripts, and any other documentation
outlining risks or precautions related to the vaccine.
COVID-19 vaccinators receive training from the Immunisation Advisory Centre (IMAC), which
includes informed consent and robust conversations around vaccine safety.
Various leaflets and flyers have been produced since 2021 to provide information to consumers on
the COVID-19 vaccines and potential side effects. Consumers are encouraged to take these home
with them after their vaccine or to take a photo of the leaflet (for ease of reference). In some
situations, it is more appropriate for consumers to receive this information verbally as part of the
informed consent process. You can find copies of pamphlets and consent forms in
Appendix One.
Additionally, Medsafe prepares safety communications to inform consumers and healthcare
professionals about safety issues with therapeutic products, i.e. medicines, including vaccines
and medical devices. Whilst many risks are identified before a medicine is used in New Zealand,
some are identified with longer use. Whenever an alert communication was published by Medsafe
regarding the COVID-19 vaccines, information regarding what symptoms to look out for was
shared with the vaccination workforce and wider health sector via newsletters and meetings.
This information was then added to the information pamphlets given to consumers when they were
vaccinated. These flyers and leaflets were regularly updated.

There are also numerous online sources of information about potential side effects from COVID-19
vaccines. This includes the Ministry of Health, and more recently the Health NZ websites, which
have a dedicated webpage about immunisation, in addition to IMAC and Medsafe’s websites.
Please note that online sources of information have been modified over time, as information about
the vaccines has been updated. We therefore cannot provide every version of information on these
sites from the start of the COVID-19 response.
