Thank you for your Official Information Act 1982 (the Act) request, received 22 July 2025, 
asking for: 
In light of recent news, I understand that Fale Andrew Lesā is a member of Child and youth 
mortality review. Could you please confirm whether he did, in fact, hold this position? If so, 
from what period? 
Also, I would like requesting all documents and information you hold pertaining to the 
appointment of Fale Andrew Lesā to this committee. 
Response 
Fale Andrew Lesā was appointed to the Child and Youth Mortality Review Committee in 
August 2016, he completed two terms on the committee as a youth advisor and consumer 
representative and his terms concluded in June 2023.  
From 1 June 2023 to June 2024 Mr Lesā agreed to become a subject matter expert for child 
and youth mortality review if required, however he has not been required and no payments 
have been made to Mr Lesā by the Health Quality and Safety Commission since June 2023. 
Attached to this letter are the appointment and extension letters to Fale Andrew Lesā. 
You are entitled, under section 28(2) of the Act, to seek an investigation and review of this 
decision by the Ombudsman.
