• The impairment assessor has provided a report 
as per the ACC User Handbook to AMA4, where

• If the rating is deferred, discussion of the 
permanence of the client's condition must sup-
port the deferral
• If the claim is managed by an accredited em-
ployer, the assessor must have also completed 
the 'ACC4152 Impairment assessment (Accre-
dited Employer) report.
