Thank you for your email on 7 November 2024, asking Health New Zealand | Te Whatu Ora for the 
following under the Of icial Information Act 1982 (the Act):  
Electronic Patient Report Form from Ambulances to DHBs and the vaccination history 
For the years 2020, 2021, 2022, 2023 I request the following information: 
I request the total EPRF number records by year from ambulance providers to hospitals.  
For each year, I would like the total number of EPRF records received that contain the word 
"vaccination" or "vaccinated" in the patient's history field.  
Kindly provide this in readable PDF or CSV format. 
Response 
The requested information is not held in a centralised location – it is held in notes on individual 
case file and on Electronic Patient Report Forms. In order to provide you with this information, 
Health NZ would have to manually review files. As such, we refuse your request under section 
18(f) of the Act. 
Please note, we have considered whether fixing a charge for the supply of the information or 
extending the timeframe for response would enable Health NZ to respond; however, we do not 
consider that either option would remove the impact that supplying the information would have on 
our other operations.
