Your Official Information Act request, reference: GOV-036068 
Thank you for your request of 20 October 2024, asking for the following information under the Official 
Information Act 1982 (the Act): 
 
Anecdotally, there has been an increase in treatment injury in the New Zealand Health System over 
the last decade. Please provide rates of treatment injury and thematic review for as far back as 
possible. Include business reports on treatment injury over the same period. 
 

On 25 October 2024, we asked you to clarify what information you were seeking in regard to the thematic 
review request. On 27 October 2024, you clarified the following: 
I'm after whatever subcategories you use to report treatment injury, so if it's mesh, or hospitals, or 
sentinel events, whatever categories you already routinely use. If you can also include the cost (in 
terms of amount paid) for those claims.

Claim counts fewer than four 
Some values in the table provided only indicate that the claim count is fewer than four (denoted as <4). In 
other instances, values are suppressed and denoted as two dots (..). This limits the potential for particular 
individuals or matters specific to certain individuals from being identified. This decision has been made 
under section 9(2)(a) of the Act. In making this decision, we have considered the public interest in making 
the information available and have determined that it does not outweigh the need to protect our clients’ 
privacy.
