Thank you for your email of 23 May 2024, to Fire and Emergency New Zealand requesting the 
following information: 
  •  …I request a copy of the safety, health and welbeing investigation report that you also 
advise was undertaken subsequent to the accident and which resulted in the safety notice 
013/2024 being published to staff and volunteers. 
 
Your request has been considered under the Official Information Act 1982 (OIA). 
 
We can confirm the safety, health and wellbeing investigation is with the report sponsor for 
review. However, as we need to make and communicate our decision on your request within 20 
working days it is necessary for us to refuse your request pursuant to section 18(e) of the OIA, as 
the document al eged to contain the information requested does not exist at this time. 
 
We note in your email of 23 May you advised you felt you had been misled regarding the existence 
of two different reports. We can assure you that it was not our intention to cause any confusion; 
rather, our intention was to ful y understand your request to provide the most accurate response 
and that seeking clarification of a request is not confirmation the requested information exists. 
 
You have the right, by way of complaint under section 28(3) of the OIA, to request an investigation 
and review of Fire and Emergency’s decision to refuse information by the Ombudsman.
