I refer to your Official Information Act 1982 (the Act) request, transferred to Te Whatu Ora on
12 December 2023, asking the following:
I am requesting under the Official Information Act that you provide me with copies of
the Covid 19 vaccination and booster authority forms that everyone was required to
complete and sign before administration of each of their original vaccinations and
subsequent boosters.
I am aware that there was a range of different Covid 19 vaccination and booster
products previously available to everyone that are no longer available, as well as
subsequently introduced and stil currently available vaccination and booster products.
Should it be the case that there historically were, and currently are, different authority
forms for the various different Covid 19 vaccination and booster products, I request that
I be provided with copies of each of them.
My preference is to have the requested information provided through the FYI website.
Response
Please find the requested information attached as Appendix A. Please note that writ en consent
was initially required for all Covid-19 immunisations, however this is no longer the case. Appendix
B is also attached for your reference.
