What Happened
under the Official Informati
Officer Notes:
Southbound Mitsubishi Pajero 
 has travelled along a straight section of road before
crossing the centreline entering a slight left hand bend. At the same time a Northbound Toyota
Soarer 
 approached the curve. An offest head on crash has occurred in the northound lane
forcing the Toyota to rotate to rest into the intersection of Hinton Road. The Mitsubishi continued and
a second head on crash has occured with a Ford Ranger 
 towing a jetski that was following
the Toyota. The Mitsubishi has rotated and flipped onto its passenger side. The single occupant was
able to self extricate before the Mitsubishi caught fire. The driver and rear right passenger in the
Toyota died of their injuries at the scene. The male passenger received moderate injuries. The driver
of the Ford received moderate injuries. The driver of the Mitsubishi received moderate injuries.

Why Crash Happened
Road & Roadside Factors:
At the time of the crash it was light, the road was dry and visibility
was clear and sunny.
The corner where the crash occurred is bordered by a maize paddock
that limits the view through the corner in both directions.
The road and roadsides are not considered causative or contributing
factors in this crash.
Road User Factors:
Mr COLLIER-WHITEWOOD crossed the centreline on a left curve
entering the opposing northbound lane into the path of the oncoming
Toyota. This is the causative factor in this crash.
Fatigue is considered to be a contributing factor in this crash for Mr COLLIER-WHITEWOOD
Ms HARPER was driving appropriately within her lane, her actions are not considered contributory or
causative factors in this crash
Vehicle Factors:
Full mechanical inspections of the Toyota, Mitsubishi and Ford
disclosed no mechanical faults that could have been causative or

contributing factors in this crash. The Mitsubishi was destroyed by
fire.
