Your Official Information Act request GOV-028422
Thank you for your request of 16 October 2023, via the FYI website, asking for information under the
Official Information Act 1982 (the Act).  
ACC requires further information from you to establish eligibility under section 12 
Section 12 of the OIA provides that a person may make a request where they are:  
a) a New Zealand Citizen; or
b) a permanent resident of New Zealand; or
c) a person who is in New Zealand; or
d) a body corporate which is incorporated in New Zealand; or
e) a body corporate which is incorporated outside of New Zealand, but which has a place of business
in New Zealand.
An original of any one of the following is acceptable as a demonstration of your eligibility: 
• A recent bill from a company who supplies your power, gas, water, phone or internet services; or
• Council rates notice; or
• Letter from the Electoral Office; or
• A copy of any New Zealand identification; or
• Anything from a government agency that includes your name and address; or
• An unexpired Rental or Tenancy agreement.

Once ACC receives this additional information from you and is satisfied that the requirements of section 12 
have been met, ACC will acknowledge your request and begin to consider its decision under the Act.
