Information Request – Evergreen Crescent Operational Report 
 
I refer to your official information request dated 1 April 2023 asking for a copy of the operational 
report for incident F3406059 which occurred at Evergreen Crescent, on 22 January 2022. 
 
Fire and Emergency’s post-incident analysis in relation to this incident is ongoing. An operational 
report will be prepared, but at this time has not yet been completed. Your request for the 
operational report is therefore refused at this time under section 18(e) of the OIA, as the 
information does not currently exist. You are welcome to make another request for this 
information at a future date.  
 
You have the right to seek an investigation and review by the Ombudsman of this decision.
