I refer to your Official Information Act request, received 29 March 2022 and extended on 12
April 2022, in which you specifically requested the following:
Could you please forward a copy of the internal review report about the Delta-infected crew of the
Mattina, that informed a Stuff article published online March 15, 2022.
Please see the attached documents in response to this request which include the minutes from
a Mattina debrief meeting held on 29 September 2021 and the results from a Public Health
Service staff survey. We think the “report” referred to in the Stuff article is in fact one or both of
these documents, as no other report or documentation exists. These documents are:

Could you please advise also, if it’s not detailed in the report, which other agencies were
involved/informed by this review.
The Mattina debrief meeting included staff from the Public Health Service and the wider
Southern District Health Board, as well as representatives from WellSouth Primary Health
Network and Southern Community Laboratories.
