Reason for investigation: Cause and origin confirmation for OIC.
Description and use of property: The property was a medium size commercial
clothing retail property. It was attached either side to other commercial buildings. It
was approximately 50m x 20m in size. There was a mezzanine area at the rear of
the shop approximately 10m x 20m. An internal stair went into this area. This
mezzanine was not open to the rest of the room as a wall covered in the
mezzanine area. This area was used for offices for the business and small break
room with kitchenette.
Scene examination: From the front (Gordon road side) of the building damage was
unnoticeable. At the rear of the property damage was visible but due to weather
covered by tarpaulin type sheets to protect the scene from weather. The roofing
iron looked affected by fire on the underside when observed. Due to the
construction of the building nothing else was visible.
Internal: Fire damage was not visible from the front of the building. Proceeding
further into the building water and fire damage was visible toward the rear of the
building. Debris was noticeable on the floor and over the racks of clothing. This
appeared to be dropped down from the ceiling above. Ceiling tiles were hanging
down in the rear right area of the building into the shop. Charring was visible on
structural timber in the ceiling above this area of the shop.
To investigate the ceiling space access was made from the mezzanine floor. An
area approximately 5m x 5m was heavily charred. With most burning in the centre
of this area above a light fitting.
The light fitting was taken down from underneath for investigation. The light fitting
particularly the remains of the ballast appeared to show extensive damage beyond
fire impingement. It was not removed for electrical testing. Electrical wiring was
also observed in this area and as much of it as was possible was investigated for
signs of arcing or similar activity. Nothing remarkable was observed on the wiring
that was investigated.
The switchboard at the front of the shop was investigated the required testing was
up to date with stickers marking the test dates. Some circuit breakers had tripped
but this could be due to the fire rather related to the cause.
The area of origin is believed to be the rear right-side ceiling space in the area of a
fluorescent light. This is because burn patterns observed on the roof trusses and
timber framing all burnt outwards from this area.
The point of origin is most likely the light ballast in the light in the area of origin.
Supposed cause: Failure of the ballast in the fluorescent light has ignited
combustible material around the fluorescent light fitting. This fire is classified
accidental.
Elimination of other causes:
The area of origin was not in an area easily accessible, so a suspicious cause of
fire was ruled unlikely as was the spontaneous combustion of chemicals as nothing
under the Official Information Act
was stored there nor could have been in an void ceiling space.
There were no external signs of lightning or similar phenomena that would indicate
this as a cause of fire. Although the weather was inclement no lightning had been
reported.
Another electrical event such as arcing of cables or rodent activity was determined
to be unlikely as no evidence could be found of this activity.
