Thank you for your request of 22 November 2021 for:
1. The investigations conducted by the Commission since 2011 where the Emergency
Locator Transmitter (ELT) failed to emit a signal as intended, specifically those
referred to in paragraph 3.44 of AO-2019-003
2. A copy of the final report referred to in paragraph 9.9 of AO-2019-003.
As you are aware, the Commission conducted an investigation into the inflight break-up of a
Robinson R22, near Mount Aspiring, 27 April 2011 (AO-2011-003). In that investigation, the
Commission made two recommendations relating to the ELT (005/14 and 006/14). We
attach a copy of that report for you.
We also
attach for you the Commission’s Watchlist:
Technologies to track and trace. This
publication details the importance the Commission places on ELT’s emitting signals as
intended and includes reference to another ELT related investigation, AO-2013-010 (report
attached).
Please note that we are unable to provide you with general ELT statistical information as we
do not hold it. We suggest the Civil Aviation Authority of New Zealand may be better placed
to assist with those queries.
With regard to your second query, the final report referred to in paragraph 9.9 is the AO-
2019-003 final report. We have
attached a copy of that report, as requested.
We are able to provide you with the above reports and Watchlist as they are publicly
available. However, please note that other investigation records are not subject to the
Official Information Act 1982 and cannot be disclosed by the Commission, except for the
