Thank you for your request under the Official Information Act 1982 (the Act) to the Ministry of Health 
(the Ministry) on 21 October 2021. You asked: 
Please advise me the AESI category that my vaccine reaction (AE0009128) has been placed 
under in Report#30. The details of the reaction are attached and as it has been reported as 
medically significant, I know it will have been classified with a cause. 
The reaction reported was not an Adverse Event of Special Interest (AESI) and the report number 
is AEFI-A-033854. Medsafe can confirm that it was reported as serious due to being medically 
significant.  
For further information, you may wish to contact Centre for Adverse Reactions and Monitoring 
(CARM) at the University of Otago directly. CARM undertakes pharmacovigilance under contract
