Information Request – F3200236 Operational Review - Vehicle Fire 
 
I refer to your official information request dated 10 October 2021 asking for the following 
information: 
 
FENZ has undertaken an Operational Review of incident F3200236.  This was a vegetation 
fire on the 15th March 2021 that resulted in the loss of an Ashburton fire appliance. 
Under the provisions of the OIA I request a copy of this operational Review. 
 
We have determined that further work relating to the review is necessary. A decision has been 
made to withhold the information requested under section 9(2)(g)(i) of the Official Information Act 
1982 to maintain the effective conduct of public affairs through the free and frank expression of 
opinions by or between or to Fire and Emergency New Zealand personnel in the course of their 
duty.  
 
You have the right to seek an investigation and review by the Ombudsman of this decision.
