Official Information Act 1982 request
Thank you for your Official Information Act 1982 (OIA) request of 18 July 2021 regarding deaths
referred to the the Coroner. Specifically, you have requested the:
1. Number of deaths reported to National Initial Investigation Office by month since
January 2016, broken down by age bracket (0-9, 10-19...80-89, 90+ etc).
2. Number of deaths Coroner took jurisdiction for by month since January 2016, broken
down by age bracket.
In response to part one of your request, please refer to Table 1 enclosed. Table 1 provides the
number of deaths reported to the National Initial Investigation Office (NIIO), by case type
and first application filing date, between 1 January 2016 and 30 June 2021. Advice cases are those
where the Coroner declines jurisdiction and a doctor certifies the death and Coronial cases are those
where the Coroner accepts jurisdiction for death.
The Ministry is unable to provide a breakdown of advice cases reported to NIIO by age bracket as the
case management system used by NIIO does not record this information. . Therefore, I am refusing
this part your request as allowed by section 18(g)(i) of the OIA as there are no grounds for believing
that the information is held by another department, Minister of the Crown, organisation, or local
authority.
In response to part two of your request, please refer to Table 2 enclosed. Table 2 provides the
number of deaths reported to the Coroner where jurisdiction was accepted, by age group
and first application filing date, between 1 January 2016 and 30 June 2021.
Please be advised that this information includes active cases which are provisional pending the
coroner’s final finding. This information is being released on behalf of the Chief Coroner.
